Unique Care Services are dedicated and highly trained leaders in the provision of care in the community.  Through our specialised training programme, we are able to offer Practical Services, Domiciliary Care and Specialised Care Services to enable people to remain as independent as possible within their own homes.

Unique Care Services emphasis on caring for people with social, health and personal care needs living in their own homes instead of residential or nursing homes, the provision of personal domiciliary care services is evolving rapidly and reflects changes at the interface between health and social care.

The care can be provided long term or for rehabilitation, some service users may require a high level of service on returning home from hospital or following a period of illness, with the correct care and support they are able to regain their previous level of independence and continue living a full and rewarding life in the luxury of their own homes.

Unique Care Services staff never undertake tasks that they feel an individual is able to do themselves or with little assistance, our emphasis is remain or regain a maximum level of independence.

Unique Care's person-centred approach to providing care and support is  important for people who receive Unique Care services (and their family or significant others) as it is to its staff. 

Unique care emphasis is always  on the person as an individual.

Person-centred care is providing care that is responsive to individual personal preferences, needs and values and assuring that patient values guide all decisions.

In a person centred approach the unique qualities of the

individual as determined by their life history and experiences, likes and dislikes, are their defining characteristics.

 

 Live@Home

The ‘Ellesmere Port & Neston Live at Home Scheme’ initiative is an element of ‘Footsteps to the Future’ that supports older people who are ‘living alone’.

This scheme offers support to older people may have become isolated from friends and neighbours due to ill health or problems getting out and about.

What we do includes:

  • Newsletter / Talking Newsletter
  • Telephone Link
  • One-to-One Befriending
  • Outings
  • Assisted Shopping
  • Welcome Centre
        (Trinity / E.Port) Mondays
  • Coffee Morning
        (Trinity / E.Port) Fridays
  • Luncheon Club
        (Churchill Court / Neston) Tuesdays, and Thursdays.

 

A New Friend
Alternatively you could receive your own personal volunteer visitor, who would be personally matched to you by our Live at Home Manager. Each volunteer is interviewed and references taken up to ensure safety and security for you, before you meet for the first time. And we obviously make sure that you have plenty in common, so that your new friendship can flourish.

    There are often different activities to take part in, it’s a good way to meet new friends and the Manager can always help you with information if you need it.
